Description:
- Provide comprehensive nutrition assessment and treatment for participants as part of an interdisciplinary team.
- Develop and implement individual and group nutrition services.
- Ensure integration into the care plan and monitor participant progress.
- Travel to various locations to deliver care and participate in team meetings.
Key Responsibilities:
- Perform assessments and evaluate nutritional status using professional guidelines.
- Calculate therapeutic diets tailored to individual needs and recommend care plans.
- Maintain participant files with diet orders, modifications, and assessments.
- Provide nutrition counseling and education to participants and caregivers.
- Coordinate with interdisciplinary team to ensure comprehensive care plans.
- Travel to participant locations to deliver direct care and conduct meal observations.
- Develop activities, diet groups, and educational programs with the Nutrition Services Team.
Requirements:
- Bachelor's Degree from an accredited Dietetics program.
- Minimum of one year of experience in clinical nutrition.
- Experience working with frail or elderly populations.
- Home care experience preferred.
- Current registration as a Registered Dietitian Nutritionist (RDN).
- Certified Dietitian Nutritionist (CDN) license in New York.
