RN Case Manager

We are pursuing a highly motivated and practiced RN Case Manager to join our team in Poughkeepsie, NY. The ideal candidate will have a passion for providing excellent patient care, strong organizational and communication expertise, and practice working in a fast-paced environment.



RN Case Manager General Requirements:

»Current New York State RN license

»Minimum of 2 years of practice in case management

»Strong organizational and time management expertise

»Excellent communication and interpersonal expertise

» capability to work independently and as part of a team

»Proficient in Microsoft Office and electronic medical records

RN Case Manager Responsibilities:

»Conduct patient assessments and develop individualized care plans

»Collaborate with interdisciplinary team members to coordinate patient care

»Monitor patient progress and adjust care plans as necessary

»Communicate effectively with patients, families, and health care providers

»Facilitate communication and coordination of care between health care providers

»Ensure compliance with all regulatory requirements and quality standards

»Maintain accurate and timely documentation of patient care activities
