Licensed Practical Nurse (LPN) travel assignment in a long-term care (LTC) setting, on the sub-acute rehab floor, located in Watertown, New York. Start date 07/06/2026 for 13 weeks. Responsibilities
- Administer prescribed medications and treatments to residents on the sub-acute rehab floor, adhering to physician orders and facility policy
- Monitor and assess residents, including those with dementia, chronic diseases, and diabetes; document changes and communicate with the supervising RN
- Perform routine nursing duties and assist residents with activities of daily living as needed
- Document care and interventions in the electronic medical record (eMAR) using PointClickCare; ensure accuracy and timeliness
- Collaborate with the interdisciplinary team to implement and adjust care plans; participate in shift handoffs to ensure continuity of care
- Maintain patient safety, adherence to infection control procedures, and compliance with regulations

Requirements
- Licensed Practical Nurse license active in New York (NY LPN)
- BLS (AHA) certification
- 1–3 years of LPN experience; SNF/LTC or sub-acute rehab floor experience preferred
- Ability to work on the sub-acute rehab floor in a long-term care setting
- NY state license required; eMAR experience with PointClickCare preferred
- Must meet the 100-mile radius eligibility rule for the assignment

Education
- Practical Nursing diploma or equivalent required
- Ongoing eligibility for state licensure, as applicable
