- NY LPN Licensure
- BLS Certification (AHA or RQI only)
- 1 year of experience as an LPN in a long-term care facility Roles & Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.

- Collect relevant data regarding residents' physical and psychosocial conditions and communicate findings to the healthcare team.

- Contribute to the development of a culturally relevant plan of resident care, promoting health and safety.

- Evaluate resident outcomes and recommend revisions to the plan of care, ensuring collaborative involvement of residents, families, and the healthcare team.

- Mentor and orient new staff while maintaining current knowledge in nursing practice.

- Ensure safety and effectiveness in the delivery of care, recognizing potential exposure to blood borne pathogens.

Compliance / Notes
- Holiday work required.

- Rotating shifts may include day/evening/night/weekend.
