Job Title: RN Profession: Registered Nurse Specialty: Home Health Duration: 260 days Shift: 5x5 days Hours per Shift: Varies Experience: Two (2) years of experience in a home health setting within the last ten (10) years are required. License: Current RN Licensure in Client is required. Certifications: BLS certification is required before the start date. Must-Have: Recent experience conducting comprehensive assessments of patients is essential. Description: All nurse assessors must meet specific requirements. Candidates must have two (2) years of experience in a home health setting within the last ten (10) years. Experience may include working in a Certified Home Health Agency, a Licensed Home Care Service Agency, or Managed Long-Term Care. Conducting assessments similar to the UAS, such as OASIS, is necessary. Providing primary care to homebound individuals is a must. Experience in care management for individuals receiving home and community-based services is required. Home care experience as a paraprofessional or Licensed Practical Nurse caring for at least two patients per week is essential. Preferred candidates will have additional relevant experience that enhances their qualifications. Experience conducting assessments in UAS-NY, especially within the last 365 days, is preferred. Caring for the geriatric population and managing chronic diseases in a home setting is beneficial. Combining home care experience as a paraprofessional or LPN with RN experience while caring for at least two patients per week is viewed favorably. Certain experiences may count toward preferred qualifications, but they do not satisfy the required qualifications alone. Home care experience with only one patient or case management limited to utilization reviews does not meet the required qualifications. All candidates should be prepared to conduct both telephonic and in-field assessments. Communication and collaboration with interdisciplinary care team members to improve care coordination and facilitate service delivery is essential. Maintaining a comprehensive working knowledge of community resources and payor requirements is required. This position requires excellent organizational, interpersonal, written, and verbal communication skills. Candidates must be able to perform comfortably in a fast-paced, deadline-oriented work environment. Additional duties may be assigned by the Regional Nurse Manager or other project management as needed.
