Overview

Job Summary
The Infection Control Coordinator is responsible for the Infection avoidance Program and tracking all infections; labs; antibiotics on an continuing basis, conducting Infection Control Audits and acting as the Antibiotic Stewardship Program Champion. The Infection Control Coordinator will be responsible for ensuring CDC; APIC; State/Federal Regulations surrounding Infection Control Standards of Practice are followed at Ferncliff Nursing Home and serves as an expert in the field of Infection Control with promotion of education and excellence in this area. The Infection Control Coordinator will be responsible for reporting Communicable diseases to the NYSDOH and the NYC DOH as required following collaboration with the Medical Director and CNO. Furthermore, the Infection Control Coordinator will be a resource for all departments to ensure compliance with Infection Control Standards all over the facility. He or she will also collaborate with medical and nursing leadership abilities to promote professional standards of care and monitor significant data and benchmarks.

Responsibilities

»Tracks all admissions and LTC Residents with Infections on an continuing basis.

»Ensures the Sepsis Protocol is initiated and followed as indicated. Reviews all return to Hospital for Sepsis to determine RCA and any additional interventions.

»Reviews IFC Data to note any trends; conducts RCA and plan of action.

»Collaborates with the Medical Director/CNO/Administrator on any communicable diseases identified as reportable. Reports to the NYSDOH/NYCDOH and ensures all tracking is completed.

»Legionella Tracking completed Monthly.

»Leads the Influenza Seasonal Roll Out for Residents and Care Members. Works cohesively with Human Resources to track all Care Members to reach a 93% Flu Vaccine Rate. Reports the Staff Influenza Numbers to the NYSDOH via HCS by May 1 of each year.

» supervises the Resident Flu/Pneumovac/Prevnar Tracking to ensure compliance; above 93%.

» supervises the Tuberculin-PPD process for Residents â€?Audits Monthly for Compliance â€?Reports to the QA Committee.

»Audits Flu/Pneumo and UTI Quality Measures Monthly for any inaccuracies/trends.

»Conducts Monthly Infection Control Audits; Joins the Environment of Care Rounds Team Weekly.

»Works collaboratively with all Departments to improve in areas identified.

»Leads the Infection Control Committee Quarterly Meetings with the Medical Director.

»Reports Infection Control Data; Trends and Antibiotic Stewardship Initiatives to the QA Committee Monthly.

»Serves as a resource for facility staff on all issues related to Infection Control avoidance.

»Provides In-Services on Infection Control Standards of Practices to Care Members in all departments; on all shifts on an continuing basis.

»Participates in the Bi-Annual expertise Fair; assists Staff Development all over the expertise Fair.

»Develops and implements standards of practice, policies, and procedures, particularly in problematic areas or areas of ambiguity.

»Regularly reviews and assesses facility Infection Control avoidance programs and processes.

»Provides support, guidance, and counseling as needed to patients and families.

»Creates monthly reports for administration and shares expertise to improve quality, efficiency, and costs.

»Minimum one (1) year practice in an Infection Control Clinical Position.

»Minimum one (1) year practice as an Infection Control avoidanceist.
