LPN ALP (Assisted Living)

SUMMARY:

Provides skilled nursing care to patients under the supervision of a Registered Nurse in accordance with CHHA philosophy, standards, and policies.

This position is employed by the Certified Home Health Agency (CHHA) and assigned to provide onsite nursing services at the Center, The LPN supports:

ALP residents in accordance with NYS Department of Health regulations for Enriched Housing/ALP; and

CHHA patients residing within the ALP, following an individualized plan of care established by the supervising RN.

The LPN functions as a clinical extension of CHHA within the ALP setting, facilitating seamless coordination, enhancing continuity of care, and ensuring compliance across both operational environments.

DUTIES
Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
Monitors patient response to treatment as per care plan and communicates any pertinent information about the patient’s clinical status and ongoing needs to their supervising RN in a timely fashion.
Will monitor and report to the RN complaints of pain. Utilizes appropriate pain management techniques as per care plan. Reinforce education to the patient and family regarding pain management.
Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Adheres to all HHA, payroll, billing and documentation policies and procedures.
Participates in HHA’s other mandated activities, as requested by Clinical Supervisor or Patient Care Services Director including in services and UR/PI committee.
Promotes patient/family instructions using various verbal and written communication techniques that consider the patient’s/family’s cultural, ethnic and/or personal needs or preferences.
Completes and submits necessary documentation in accordance with HHA policy and applicable regulations.

II. ALP RESIDENT CARE RESPONSIBILITIES

Assists residents with medications and treatments as prescribed and in compliance with DOH ALP regulations.
Writes and transcribes physician orders per approved protocol.
Supervises paraprofessional and auxiliary staff in the ALP as directed.
Conducts daily rounds to monitor condition changes.
Assists with resident admissions, discharges, transfers, and hospital transitions.
Observes, records, and reports symptoms, reactions, or condition changes to supervising RN and ALP Director.
Maintains accurate ALP progress notes, documentation, and incident reporting.
Ensures infection-control and risk-prevention standards are consistently upheld.
Supports residents during physician visits and family communications.

Health promotion responsibilities (ALP-CHHA Cross-Function)

Provides patient awareness and instruction to residents and families on medication management, disease prevention, chronic-condition awareness, fall prevention, and self-care strategies.
Promotes wellness and independence through reviewed and reinforced education and supportive engagement.
Reinforces clinician-developed teaching plans and monitors resident understanding and adherence.
Provides emotional support, reassurance, and companionship to promote a stable, nurturing, and person-centered environment.

Encourages resident independence while ensuring safety and respect for personal preferences.

III. JOINT ALP–CHHA CARE COORDINATION

Coordinates care for CHHA patients residing in the ALP, ensuring care plans between ALP and CHHA are aligned and workflows are synchronized.
Communicates clinical updates to CHHA RN Case Managers, ALP Director, DPS, and the interdisciplinary team.
Identifies ALP residents who may benefit from CHHA services and communicates referrals to Intake and Clinical teams.
Ensures documentation supports accurate CHHA billing and ALP compliance.

IV. CHHA Patient Assignment Parameters

To ensure safe, effective, and regulation-compliant service delivery within the Assisted Living Program (ALP) environment, the LPN will be assigned a balanced caseload of CHHA patients residing in the ALP. Based on clinical workflow, ALP census, and operational demands, the optimal daily CHHA patient assignment is three (3) to six (6) patients per shift.

This range supports:

  • Resident safety and timely ALP medication administration

  • Completion of CHHA patient care tasks within scope and supervision of the RN

  • Consistent documentation and communication across both programs

  • Appropriate time allocation for rounds, ADLs support, incident follow-up, and emergent resident needs

  • Regulatory compliance for both CHHA and ALP service lines

Caseload volume may be adjusted as needed based on patient acuity, ALP census, clinical complexity, or direction from the supervising RN, DPS, or ALP Director.

V. PROFESSIONAL CONDUCT & COMPLIANCE

Demonstrates professional conduct, effective communication, and collaborative teamwork.

Maintains confidentiality, HIPAA compliance, and ethical clinical practice at all times.

Participates in mandatory in-services, QAPI activities, competencies, and DOH-required trainings.
Follows all policies and directives issued by the DPS, ALP Director, and supervising

QUALIFICATION: LPN, with 1 yr exp in skilled nursing/ Assisted Living or CHHA
Currently licensed as a Licensed Practical Nurse

Has recent community health care/acute care experience.

Physically and mentally capable of performing job responsibilities.

Has knowledge of Medicare and Medicaid regulations
