Schedule: Monday–Friday | 8:30 AM – 5:00 PM

Hours: 40 hours/week

Position Overview

A healthcare is seeking a Licensed Social Worker (LMSW) for a short-term contract assignment. The Social Worker will provide psychosocial assessment, care coordination, and discharge planning support to patients while collaborating closely with interdisciplinary healthcare teams.

Responsibilities

• Conduct psychosocial assessments and develop patient care plans

• Provide counseling, crisis intervention, and emotional support to patients and families

• Coordinate discharge planning and connect patients with community resources and social services

• Advocate for patients’ psychosocial needs within the healthcare system

• Collaborate with physicians, nurses, case managers, and ancillary staff

• Maintain accurate and timely documentation in the Electronic Medical Record (EMR)

• Manage a high-volume caseload in a fast-paced clinical environment

• Ensure compliance with hospital policies, regulatory standards, and ethical guidelines

Compensation

• Gross Weekly Pay: $1,900/week

• W2 Rate: $18/hour

• Weekly Stipend: $1,225/week

Required Qualifications

• Master’s degree in Social Work (MSW) from an accredited institution

• Active New York State LMSW license (required)

• Minimum 6 months to 1 year of experience in a medical or healthcare setting

• Strong knowledge of community resources and social service programs

• Excellent communication, assessment, and interpersonal skills

• Ability to manage a large caseload effectively

• Epic EMR experience preferred

• Proficiency with Microsoft Office Suite
