TravelNurseSource is working with Lancesoft Inc to find a qualified Home Health RN in Poughkeepsie, New York, 12601!

Health Quest Homecare is seeking a dedicated Registered Nurse to provide high-quality clinical care to patients within their homes. This role requires a high degree of independence and the ability to manage a diverse patient load across a designated territory in Ulster and Dutchess Counties.

Compensation & Logistics

• Guaranteed Hours: 40 hours per week
• Travel: Required (Estimated 50–75 miles per day)
• Equipment: Must have a reliable vehicle in good working condition

Key Responsibilities

• Provide comprehensive home care for patients with varied medical diagnoses.
• Clinical Skills: Wound care, Wound Vac management, PleurX drains, dressing changes, and ostomy care.
• Specialized Care: PICC line maintenance, nephrostomy tubes, and venipuncture/blood draws.
• Chronic Disease Management: Patient education and clinical oversight for Diabetes and Congestive Heart Failure (CHF).
• Operate independently in diverse home environments while maintaining high standards of care.

• Work Site: 2649 South Road, Suite 220, Poughkeepsie, NY.
• Territory: You will be assigned a specific territory within the Ulster and Dutchess county areas.
• Independence: This position is ideal for nurses who enjoy practicing independently and traveling between patient sites.

Requirements

Experience

• Home Care: 1–3+ years of dedicated home care experience required.
• Clinical: At least 1 year of recent clinical experience.
• Technical: Experience with HCHB (Homecare Homebase) application is highly preferred.

Certifications & Licenses

• Active New York (NY) RN License .
• Current BLS certification.
