Listing description We are currently seeking a compassionate and skilled Certified Home Health Aide (HHA) to join our dedicated team in Carmel, NY. As a Certified HHA, you will provide personal care, home management, and other related home health supportive services to individuals who require assistance in their home environment due to illness, discapability, social disadvantage, or other challenges.

Certified Home Health Aide (HHA) General Requirements:

»Valid certification as a Home Health Aide (HHA) by the State of New York.

»Current MDC (Medical Documentation Certificate) Updated.

» capability to work the following shifts: 3pm-11pm and 11pm-7am.

»Excellent communication and interpersonal expertise.

»Compassionate and patient nature.

» capability to follow instructions and work independently.

»Physical stamina and strength to assist with lifting, transferring, and other tasks as required.

»Reliable transportation to commute to clients' homes.

Certified Home Health Aide (HHA) Responsibilities:

»Assist individuals in their everyday activities, including personal hygiene, dressing, and grooming.

»Provide support in managing household tasks, such as meal preparation, light housekeeping, and laundry.

»Offer companionship and emotional support to promote the well-being of clients.

»Help with mobility and transportation, as needed.

»Monitor and document changes in clients' health status.

»Communicate effectively with clients, their families, and the healthcare team.

»Adhere to all significant policies, procedures, and care plans.

»Work under the direct supervision of a licensed nurse.
