We are seeking a dedicated Licensed Practical Nurse to provide quality patient care during evening shifts at a skilled nursing facility in Queens, NY. The ideal candidate will bring compassion and clinical expertise to support our residents' health and well-being.

Qualifications:

• Education: Graduate of an accredited practical nursing program.
• Licensure: Active New York State LPN license.
• Experience: Must have a minimum of 1 year of experience in a nursing home setting.
• Technical Skill: Proficiency in patient care documentation and medication administration.
• Soft Skills: Compassionate caregiver with strong clinical judgment and teamwork abilities.
• Other requirements: Must be immune to Hep B.

Key Responsibilities:

• Provide direct nursing care to residents during night shifts.
• Administer medications and treatments as prescribed.
• Monitor and document patient conditions and vital signs.
• Collaborate with healthcare team members.
• Maintain accurate patient records and reports.
• Ensure compliance with facility policies and procedures.
