Description:
- We are seeking a Registered Nurse for a Day Habilitation Program.
- This role involves supporting individuals with developmental disabilities in a structured residential setting.
- The position is part-time and located onsite in Brooklyn, NY, requiring close collaboration with interdisciplinary teams to manage the medical needs of residents.
Requirements:
- Conduct comprehensive nursing assessments
- Develop individualized care plans
- Monitor and evaluate medical needs
- Maintain clinical documentation
- Coordinate care with healthcare professionals
- Supervise and support AMAP staff
- Participate in treatment planning meetings
Qualifications:
- Active NYS RN license
- Experience in residential or OPWDD settings preferred
- Experience with adult populations and developmental disabilities preferred
- Strong documentation skills
- Effective care coordination skills
