About the Position
- Deliver high-quality nursing care to patients in an acute rehab setting.
- Assess, plan, implement, and evaluate individualized patient care plans.
- Monitor neurological status and recognize changes in patient condition.
- Administer medications and treatments per provider orders.
- Assist with patient mobility and transfers using equipment such as Hoyer lifts and sit-to-stand lifts.
- Collaborate with physical therapy, occupational therapy, and other disciplines to promote optimal recovery.
- Document all patient care and progress accurately in Epic (preferred).
- Educate patients and families on care plans, safety, and discharge goals.
- Maintain infection control and safety standards in compliance with hospital policies.
Requirements
- License: Active NY RN license (required)
- Experience: Minimum 1 year of recent acute rehab or hospital experience
- Certification: BLS (required)
- Charting System: Epic (preferred)
- Patient Ratio: Days 1:5 | Nights 1:12
- Float Requirement: Yes – Medical/Surgical units
- Weekend Coverage: Every other weekend
