Responsibilities
- Deliver skilled nursing care to adult and geriatric residents, including those with degenerative or chronic conditions.
- Administer medications, monitor vital signs, and perform routine treatments per individualized care plans.
- Recognize changes in patient condition and escalate concerns to the interdisciplinary team.
- Document care activities and patient responses in EPIC with accuracy and timeliness.
- Collaborate with Physical Therapy, Radiology, Social Services, and Pharmacy to support comprehensive resident care.
- Manage behavior concerns using de-escalation techniques and adhere to safety and restraint policies as required.
- Obtain 12-lead ECG/ECG as ordered and support pain assessment and comfort measures.

Requirements
- Active New York LPN license (single-state, no pending licenses)
- BLS certification
- Minimum 1 year of clinical experience; nursing facility/ skilled nursing facility experience required; LTAC experience preferred; prior charge nurse experience is a plus
- EPIC charting experience
- Ability to work in a long-term care setting with typical patient ratios
- Willingness to travel or work a travel assignment; first-time travelers accepted
- Fluency in providing care for adults and geriatric populations; ability to float within the skillset as needed
