Personal Touch Home Care is expanding our Special Needs Program and seeking compassionate Physical Therapists who are passionate about delivering high-quality care in the home setting.
This is a fee-for-service position, paid per visit and this is a 1099, contracted position. In order to be considered for this position, you MUST have a valid and active license in the state of NY.

Pay: $100.00 per visit

Job Overview: As a Physical Therapist with Personal Touch Home Care, you'll play a crucial role in providing direct physical therapy services based on the physician's plan of care. You will also provide rehabilitative services to patients who are unable to travel to a clinic or hospital. This is focused on improving patients' quality of life by promoting mobility, function, and independence within the comfort and convenience of their own homes. Your responsibilities will include:
Assessing and evaluating the therapeutic, rehabilitative, and functional status of patients.
Participating in the development of comprehensive care plans tailored to each patient's needs.
Evaluating the home environment to ensure it supports the patient's rehabilitation goals.
Assessing muscle strength, mobility, gait, range of motion, and the patient's potential for rehabilitation.
Instructing patients/caregivers in the use and care of therapeutic appliances to support their rehabilitation journey.
Maintaining detailed records of patient assessments, treatments provided, progress notes, and discharge plans.
Qualifications:
Licensed to practice a physical therapist within the State
Possesses a degree from a baccalaureate or master’s program in physical therapy approved by The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association or The Council on Medical Education of the American Medical Association.
Prefer two (2) years of appropriate experience as a physical therapist. Home health experience is preferred.
Demonstrates good verbal and written communication and organization skills.
