Are you a patient-centered healthcare professional committed to delivering high-quality, respectful care to a diverse patient population? Callen-Lorde is seeking a Primary Care Provider (PCP) to join our collaborative, multidisciplinary team.

As a PCP at Callen-Lorde, you key responsibilities will include:

Clinical Care

• Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
• Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines.
• Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals.
• Document all care promptly in the electronic health record (EPIC).
• Provide primary care services, including care for patients living with HIV and those seeking gender-affirming services (training available if new to these areas).

Collaboration & Coordination

• Make appropriate internal and external referrals.
• Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care.

Quality & Professional Development

• Participate in quality improvement initiatives and case conferences.
• Engage in ongoing professional development opportunities, including continuing medical education.

Work Schedule

• Three morning/daytime shifts: 8:30 AM 4:30 PM
• Two evening shifts: 12:00 PM 8:00 PM
• Optional Saturdays: 9:00 AM 3:00 PM
  

Compensation

The salary range for this position will be commensurate with the experience and qualifications of the selected candidate.

• Nurse Practitioners $127,491.00 - $151,843.82
• Physicians (MD/DO) Family Medicine and Pediatrics $200,000.00 - $260,000.00
• Physicians (MD/DO) Internal Medicine $205,000.00 265,000.00

Additional differential of $10,000 annually for obtaining and maintaining Board Certifications in the areas of MDs and DO's - Infectious Disease, Endocrinology, Geriatrics, and Adolescence Medicine. This role(s) is a part of 1199 SEIU collective bargaining agreement

• New York State Certification/Licensure (MD, DO, or NP)
• Current NYS Registration and DEA Controlled Substances Registration Certificate
• Current CPR Certification
• Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease (for MDs/DOs)
• Experience in community-based primary care preferred
