In this role, you will:

• Support the development and ongoing revisions to individual Staff Action Service Plan by providing input and information.
• Track individual progress towards desired goals and objectives in Life Plan.
• Segment job duties and assignments into specific steps to facilitate the learning of the people we serve.
• Monitor individual level of performance including determining when support can be maintained, increased, or gradually faded.
• Represent agency in a professional and exemplary manner, while in assigned community worksites. Serve as a liaison between people we serve and worksite employees/employers to facilitate appropriate work readiness skills training and professional work relationships.
• Maintain accurate records on job coaching services provided to each person we serve.
• Respond to crisis situations in a humane and proactive manner and in accordance to policy procedures and training.
• Develop and maintain written documentation, as requested.

We require:

• High School Diploma or equivalent
• Clean NYS Driver’s License
• At least six (6) months employment experience
• Good math and communication skills
