Job Description
The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better address racial health gaps and improve health outcomes for all New Yorkers. The divisions Deputy Commissioner also serves as the Agency's Chief Medical Officer. The Bureaus of Neighborhood Health (BNH) were developed with special focus to address health equity in the neighborhoods of the South Bronx, Central Brooklyn and East & Central Harlem. The BNHs operate the Neighborhood Health Action Centers which apply a placed-based community centered approach in improving health outcomes and mitigating the impact of policies that exacerbate health inequity.
The Bureau of Bronx Neighborhood Health seeks to hire five (5) Community Health Workers.
DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

Work closely with colleagues in the Division of Environmental to become versed on latest climate agency objectives.

Participate in Just in Time training with DOHMH divisions that serve as a lead for a local/hyperlocal emergency response.

Distribute information and coordinate information sessions to community residents and businesses in response to emergency incidents.

Maintain communication with local organizations and stakeholders to ensure alignment in emergency planning and response efforts.

Support agency efforts in building resilience in identified communities to mitigate impact from localized emergencies such as legionella outbreaks; conduct messaging, communicating and other efforts to make community aware of climate health preventive measures.

Engage with community partners to promote health services, resources, and education.

Develop and maintain relationships with a broad variety of community partners.

Represent the Bureaus of Bronx, Brooklyn, or East Harlem Neighborhood Health at health fairs, meetings, and other external events occurring throughout the community.

Deliver presentations and workshops to diverse audiences throughout the community.

Maintain and report data that demonstrate community activities and results.

Support Bureau-wide programs and initiatives.
PREFERRED SKILLS:
2+ years of community work experience.Demonstrated capability to lead implementation of special community level initiatives.
Excellent organizational skills and highly detail oriented.
Excellent oral and written communication skills.
Demonstrated ability to effectively establish and maintain working relationships with community groups.
Knowledge of Bronx, Brooklyn, and East Harlem local communities and local organization leadership.
Comfortable speaking in public
Bilingual Spanish, French and Chinese language proficiency encouraged to apply.

Qualifications
A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
