OSH Asthma Program Outreach Coordinator
Job Description
The Office of School Health (OSH) is a joint program of the Department of Education and the Department of Health and Mental Hygiene responsible for promoting the health of 1.3 million school aged children enrolled in approximately 1,800 public and non-public schools in New York City. This is achieved through a combination of public health initiatives, case management, education, and direct clinical services. The Asthma Case Management Program (ACMP) isa comprehensive school-based initiative embedded in the OSH Medical Unit that provides individualized care coordination, education, and follow up for students with uncontrolled asthma. The program partners with families, school staff, and community healthcare providers to reduce asthma-related emergencies, improve daily symptom control, and support student attendance and academic success.
Duties will include but not be limited to:

Coordinate with community-based organizations involved in community schools to facilitate the provision of services.

Develop and administer an internship program within OSH ACMP in collaboration with NYC colleges and universities.

Collaborate and develop strong relationships with local community-based organizations providing asthma case management services in communities to share best practices and link students and families enrolled in ACMP to wrap-around services.

Update and maintain resource lists for ACMP case managers.

Provide presentations, workshops, and trainings to community schools to parents, teachers, and students.

Provide direct supervision to interns in schools to work within the school community and provide additional outreach services.

Will be assigned to a field site when needed to coordinate/assist with conferences/presentations/ trainings.

Prepares report and other written materials

Assists in the evaluation and strategic planning of programs and presentations.

Qualifications
A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
